Brain Pituitary tumour pre op




 

Radiological Report :

Clinical History : Clinically and biochemically has Acromegaly.

MRI Head : There is a large lobulated mass arising from an expanded sella with a component extending into the left middle fossa and a smaller lobule extending back to impinge on the brainstem. In the midline sagittal section the tumour is 47mm in height, 38mm AP dimension and impinges on the optic chiasm.

The component extending to the left indents the temporal lobe and indents the mass of the brain extending close to the insular cortex. This component has a maximum 36mm diameter on the axial images and indents the inferior part of the lentiform nucleus extending up close to the left cerebral peduncle.

A further lobule extending posteriorly in the midline lie in contact with the upper pons. There is local mass effect around the base of the brain but no sign of any hydrocephalus. In the parasellar region tumour seems to wrap around lateral to the internal carotid artery but there is no sign of any significant carotid stenosis.

Expansion inferiorly produces a substantial mass extending into the nasopharynx.
